Joining/Leaving — National Health Insurance

In Japan, enrolling the insurance (whether National Health Insurance or Social Insurance) is required to all resident, except the ones under the social welfare.
Please notify us in following occasions (within 14 days!!) and bring your seal.
Joining the National Health Insurance
- When you moved in to Naha from other city, ward or village.
- Notification of change of residence (Tenshutu Shoumei-sho) is required.
- When you quit/leave the insurance at work.
- Notification of loss of the social health insurance qualification (Kenkou Hoken Shikaku Soushitu Shoumei-sho, issued when you leave the insurance at work) is required.
- When baby is born.
- National Health Insurance Certificate and Maternal and Child Health Handbook (Boshi Kenkou Techo 母子健康手帳) are required.
- When leaving the social welfare.
- Notification of discontinuance of social welfare (Hogo Haishi Kettei Tsuuchi-sho) is required.
- When alien resident moved in to Naha city.
- Alien Registration Card (Gaikokujin Toroku Shomeisho) is required.
*Alien residents who have valid visa to stay in Japan for at least a year are eligible to enroll the National Health Insurance.
Alien residents may be eligible even the visa is valid less than a year, depending on the purpose of the entry.

Leaving the National Health Insurance
- When moving out to other city, ward or village.
- National Health Insurance card is required.
- When joining the insurance at work or became dependent.
- National Health Insurance Certificate and Insurance Card (or Kenkou Hoken Shikaku Shutoku Shoumei-sho) from work are required.
- When one starts receiving social welfare.
- National Health Insurance card and notification of social welfare start (Hogo Kaishi Kettei Tsuuchi-sho) are required.
- When person insured passed away.
- Death certificate or notification that proves person’s death is required.
Other
- Address changed within the city/Changes of name or head of a household…
- Please bring the National Health Insurance Certificate.
- Loss of the National Health Insurance Certificate…
- Please bring your ID.
- When dependent will leave and resides in another city or ward for schooling…
- Please bring the National Health Insurance Certificate and Certificate of Student Registration.
- When National Health Insurance participants leave the NHI and join the Social Insurance at work, former participants lose the qualification and can not use the National Health Insurance Certificate.

If participants accidentally use the certificate to consult a doctor, participants must pay the full price.
Please do not forget to return the certificate when leave the NHI.
