Infant Medical Cost Subsidy

Until When?

• Until the last day of the month of child’s 3rd birthday, subsidize apply for both hospitalization and outpatient.
  • (If the child was born in May 10th, subsidy applies until end of the May)
• From a month after the child turns three full years, until the last day of the month of child’s 6th birthday, subsidize apply for hospitalization.

Required Items

• Health Insurance Card
• Bankbook (Yokin Tsucho 預金通帳)
• Seal (Inkan 印鑑)

1. You must complete the procedure before the last day of month in child’s 3rd birthday. 
2. Certificate of qualification of recipient (Jyukyu Shikaku Nintei-sho 受給資格認定証) will be issued when procedures completed. 
3. When child reached three full years, another procedure is necessary to apply subsidy, before child reached six full years.
4. The medical expense that passed a year since initial visit cannot be refund, even through you qualify for subsidy.
5. If certificate of qualification is not issued, you would not be able to receive.

When term expires (which is a year after initial visit), you could not apply for the qualification of recipient
