In Japanese law, it is required to register the birth of baby with 14 days.
When baby is born, please register the birth of your baby at Citizen’s Affairs Division (Shimin-ka) in Main Office or one of the branch offices.
Registration must be notify by
- Father or mother of the baby
Place of Registration
Please register at one of the Municipal Office
- Municipal where the baby was born
- Municipal where the parents have their address
- Municipal where the parents have their domicile of origin (HONSEKI, if child possesses Japanese nationality)
Required Papers
- Birth Notification Form (Shussyou Todokede-sho, you can pick up a copy at counter of Citizens’ Affairs Division)
- Birth Certificate (Shussyou Shoumei-sho, issued by the doctor or obstetric nurse)
- Seal or signature of notifier
- Maternal and Child Health Handbook (Boshi Kenkou Techou)
- Certificate of National Health Insurance (If you are member)
Name must be registered in Chinese character designated for naming and everyday use (Jyouyou Kanji or Jinmei Kanji) or Hiragana or Katakana.
If newborn child does not possess Japanese nationality, parents must register the baby as alien resident and acquire the status of residence.
- Alien Registration:
- Register within 60 days.
- Immigration Office:
- Request the status of residence within 30 days.
When baby is born, please notify the embassy of your country.
